Accident/Incident Report
An Accident/ Incident report will be completed if a child is injured or to communicate an incident at our program. Staff will share the report with the person picking up the child and have it signed by the adult. Staff will complete the report to include all information on the form describing the accident/incident, injury, care of injury etc. If your child receives a head injury, staff will call to inform you of the accident. In this case please watch your child for signs of concussion. Head injuries can be a very serious matter. For more information on concussion management please go to www.cdc.gov
If your child is seen at the clinic, hospital or dental office as a result of an injury at Adventure Center, you must contact Pat Bourke at 763-689-6180 after treatment. We are required to report serious injuries to DHS within 24 hours.
